What should I bring to my appointment?

The following items should be brought to your appointment:
- Current insurance card
- Copayment, if applicable (Cash, Check, Mastercard, Visa, Discover)
- Loose fitting clothing free of metal (for x-rays)
- X-ray or MRI films, if the test was performed somewhere other than (Bronson Hospital, Borgess Hospital, or Southwest Michigan Imaging -KNI)
- Current Medication list or medication bottles
