  • Insurance Information
    • If you do not have insurance, payment in full will be requested at the time of service.
    • Please bring your insurance card to each visit and notify us of any changes to your policy.  This will allow us to submit a clean claim on your behalf.
    • Be prepared to pay your copay and/or any unpaid balance at each visit.  Patient statements are mailed once a month for any unpaid balances on your account.  Payment will be expected within 20 days and will be applied to your oldest balance first.
    • Balances left unpaid will be subject to collection fees.

  • Medicare Advantage Plans

    Effective January 1, 2010; there are some major changes occurring with the Medicare Advantage Plans.  As an insurance subscriber, it is your responsibility to understand how these changes will affect you.  Beginning January 1, 2010; your healthcare provider must have a participating agreement with your Advantage Plan in order for you to receive the highest benefits available.  If you are unsure about what this means, please contact your insurance company at the phone number printed on your card.

    Beginning January 1, 2010; we will only participate with the following Medicare Advantage Plans:

    Medicare Plus Blue
    Blue Care Network Advantage
    Priority Health Medicare Advantage
    United Healthcare Advantage
    Humana Gold
